What types insurance do you accept?
All major insurance plans are accepted. We are in-network with Blue Cross Blue Shield, United Healthcare, Aetna, Humana, and Medicare. Your coverage and benefits are dependent on your individual insurance plan.
Can I fill out paperwork/forms ahead of time?
Yes! Use our "New Patient Forms" button to download and print the paperwork. You can complete the forms and bring them to the office at the time of your first appointment. If you do not have access to a printer, please arrive a few minutes early to fill out the necessary paperwork.
I'd like to book an appointment online. What type of appointment should I choose?
Our online booking system offers a "new patient visit" option for individuals who are new to the office. If you have never been a patient at the office previously, this is the correct booking selection for you.
The "existing patient new injury visit" option is for those who have been seen at the office previously and are experiencing a new type of pain, new location of pain, or new injury. This selection is also appropriate for those who have not been seen in the office for more than 3 months.
The "basic visit" is for current patients only. This includes a brief assessment and chiropractic adjustment.
The "comprehensive treatment visit" is for current patients only. This visit includes brief assessment as well as chiropractic adjustments along with other therapies.
